Submitting the Disability Tax Credit

If you provided your medical practitioner with a reference number, they will be able to submit it on your behalf. Have a conversation with your medical practitioner if you would like to review your application before submission.

If you did not provide your medical practitioner with a reference number, you can submit your completed application online (form T2201) by either 1) logging in to your CRA My Account and uploading the application through the “Submit Documents” function, or by 2) physically mailing the application to the closest tax centre.
