Once you have completed Part A of the form, book an appointment with your medical practitioner to complete Part B of the form. Your medical practitioner may complete the form using the paper application or an online application. They may charge a fee for their support. You may be able to claim this amount as a medical expense when you file your taxes.
It is always important to be prepared for an appointment with your practitioner. Read through the application and provide specific examples of your restrictions in the different activities listed. This will help your medical practitioner know what kind of information they can include to strengthen your application.
