Preparing to complete Part A of the DTC application
Once you have a medical practitioner in mind who can certify the restriction, you can start Part A of the application.
Part A is the part that’s for the individual applying for the Disability Tax Credit. Part B is for the medical practitioner to fill in.
There are three main ways to fill in Part A:

Online: This needs a Canada Revenue Agency (CRA) My Account. If you file your taxes online, you likely have a CRA My Account.

Over the phone: This involves phoning the CRA or using the teletypewriter system.

Paper form: This is a PDF you can fill out, either on your computer or by printing it and filling it in by hand. You can then sign it.
Note: You do not need to complete Part A of the application before completing Part B with your medical practitioner. You can complete Part A any time before you submit the application. However, filling out Part A online or over the phone ahead of time can allow your medical practitioner to submit the application on your behalf right after they complete it.

If you complete Part A of the DTC application online or over the phone, you will be given a reference number. Keep this to give to your medical practitioner if you would like them to submit the DTC on your behalf. If you prefer to submit the application yourself, you do not need to provide your practitioner with a reference number.
