Finding a medical practitioner to certify the Disability Tax Credit
There are a number of medical practitioners that may be able to certify your form. Medical doctors and nurse practitioners can help fill out all sections of the form.
Depending on your restriction, you can also ask a more specialized healthcare provider, such as an optometrist for vision restrictions. We recommend that, if possible, ask the medical provider that knows your restrictions well.
If you don’t have a family doctor, you may want to start by going to a walk-in clinic or finding out how you can be added to a waiting list for a physician accepting patients. In BC, you can visit the Health Connect Registry for help finding a family doctor or nurse practitioner.

Who can certify what restrictions?
- Medical doctor (e.g. your family doctor): All restrictions
- Nurse practitioner: All restrictions
- Optometrist: Vision
- Audiologist: Hearing
- Occupational therapist: Walking, feeding (eating or preparing meals), dressing
- Physiotherapist: Walking
- Psychologist: Mental functions
- Speech-language pathologist: Speaking
Those with restrictions in physical and mental functioning categories might consider asking a medical doctor or nurse practitioner to fill out the form. If necessary, the CRA accepts multiple applications for different categories.
