Help patients apply for the Disability Tax Credit

Filling out Part B of the application can be tricky because the form asks questions about your patients’ restrictions in their daily lives that you may not be privy to.

When filling out the application, remember that eligibility is not based on diagnosis. It’s more important to detail how symptoms can lead to restrictions in basic daily activities.

You may feel like adding medical notes already prepared to help to strengthen the application. Sometimes, these notes can add confusion to the adjudicating process. Be sure to check if these notes are clear, and do not include information that will contradict restrictions listed in the application.

Overview of the process

Step 1: Fill out Part B of the form by detailing specific examples of restrictions in your patient’s daily life.

Step 2: Fill out the “Certification” section, the last section of the form. The most important question of this section is question 2 which asks if you have all the medical information necessary to certify all years your patient is applying for. This question helps to determine the duration of your patient’s prior eligibility.

  • If you check “no” to this question, your patient will only qualify for the years they became your patient. Being eligible for all the years they were restricted will help your patient get financial support for all the prior years they were eligible.
  • If you feel uncomfortable checking “yes,” speak to your patient about what information you might need to certify the form. Feel free to consult your patients’ other practitioners for more medical information that can support the application.

Step 3: Return the form to your patient for them to review and submit by mail or online through their CRA MyAccount. You can also submit the application on their behalf if filling out Part B of the application online.

Why is it important to get your patient’s perspective when filling out this form?

For a number of reasons, you may not be aware of all the daily restrictions your client experiences at home. Some patients might feel like they have too many restrictions to talk about at an appointment, others might have struggled previously to disclose their disabilities. This tool can help bridge the gaps with these conversations.

While people with disabilities are often asked to prove their disability to access social programs, we hope that using a person-centred approach will help health practitioners and patients to work together to fill out forms that can deeply impact a person’s health and financial wellbeing.
